I do not understand the question

Qu'est-ce qui est arrivé à Elsa ? - how does this translate to 'what happened to Elsa?' This is from the test questions. Kindly clarify.

This is a colloquial use of the verb "arriver" here.
Indeed, in French, you can use "arriver" in the sense of "to happen", as such:
"Ces choses-là arrivent !" (Those things happen!)
or
"Qu'est-ce qui t'est arrivé ?" (What happened to you?)
